Photo Release Consent Terms of Services

I hereby authorize YES to the Book, Inc. to publish the uploaded videos and/or photographs of myself and/or the minor child or children listed above, and our names and likeness, for use in the YES to the Book, Inc. print, website, and video-based marketing materials, as well as other YES to the Book, Inc. publications.

I hereby release and hold harmless YES to the Book, Inc.from any reasonable expectation of privacy or confidentiality for myself and/or the minor child and children listed above associated with the images uploaded above.

Further, I attest that I am the parent or legal guardian of the child or children listed above and that I have full authority to consent and authorize YES to the Book, Inc. to use their likenesses and names.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation for any type associated with the receiving or publication of the uploaded videos and photographs in YES to the Book, Inc. marketing materials or other publications.

I hereby release YES to the Book, Inc., its contractors, its employees, board members, and any third parties involved in the creation or publication of YES to the Book, Inc. publications, from liability for any claims by me or any third party in connection with my participation or the participation of the minor child and/or children listed above.
